HCRG Care Group originated in around 2007 as the Assura Medical division of property company Assura Group.

In 2010, a majority share was purchased by Virgin Group, and by 2012, it was a separate company under the Virgin Care brand.

358 surgeries were listed as being involved in mid-2012, each with a 50% ownership split between the surgery GPs and Virgin.

In October 2012, the company announced it would take over all jointly owned GP-provider companies to avoid conflict of interest with clinical commissioning groups.

The company made bids for contracts put out by clinical commissioning groups, NHS England, and local authorities, and has contracts stretching for more than a decade with a total potential value of £2bn as of 2018.

HCRG Care Group was acquired by Twenty20 Capital, a privately owned investor, in December 2021 for an undisclosed sum.

The company was immediately rebranded as HCRG Care Group, and in March, it attempted to use a court-ordered injunction to compel a cybersecurity breach reporting website to remove references to examples of data taken from its systems.

From 1 April 2025, HCRG Care Group will take over community-based care services in Bath and North East Somerset, Swindon, and Wiltshire.

The transfer of services will be done in a way that minimizes disruption to patients, with the same familiar faces continuing to provide care. Some colleagues may wear new uniforms and carry updated ID lanyards.

The new partnership between HCRG, the NHS, local authorities, and charities will aim to deliver better outcomes for the local population and provide greater support for people to live healthier lives. This is in line with the government's aims of moving from hospital care to community care.

The neighbourhood health guidelines published by NHS England require a new way of working for the NHS, local government, social care, and their partners. This will enable care and treatment to be joined up and delivered close to people's homes.

The handover of services has been planned to take place as smoothly and seamlessly as possible, with no interruption to patient-facing services. This has involved close work between the ICB, HCRG, and previous providers over the last six months.
