If you use an out-of-network provider, submit a BCBSIL/EyeMed vision claim form to First American Administrators, Inc. at PO Box 8504, Mason, OH 45040-7111 for reimbursement.
You'll need to create a new EyeMed online and/or mobile app account after receiving the BCBSIL ID card, and use a different email address from your current/past EyeMed account.
